The line is crossed two ways. The repair grows past about a ten-by-ten area, or it involves decking, because the rot under a long-running leak has to come out. At that point the repair is inspected like a re-roof. A contractor should tell you before opening the roof which side of the line the job is likely to land on, and again once the decking is visible.

The stain on the ceiling is rarely under the hole. Water enters at the flashing, runs down the top of the decking or along a rafter, and drips where gravity finally wins. A good leak repair starts in the attic with a flashlight on a dry day, follows the staining back uphill, and only then goes on the roof. Skipping the attic is how a homeowner pays for the same repair twice.
